mkfadumprd: drop unset globals from debug output

mkfadumprd doesn't call prepare_kdump_bootinfo from kdump-lib.sh. Thus
both KDUMP_KERNELVER and DEFAULT_INITRD are always empty. Simply remove
them from the debug print.
