Norman Walsh's schemas (DTD, Relax NG, W3C schema) for Docbook 5.X

File `db4-entities.pl` has to be removed because we can not be sure about it's license. Even if we implied the same license as other parts of the package, the LicenseRef-OASIS-spec is only allowed for content. This file is a script, not content. It can not be packaged. The howto upgrade guide, that is also removed by this, has unspecified license. Currently we are not able to determine in a provable way the license with which are these files licensed with.
